i am currently working on offering in-network insurance for select providers in washington state. in the meantime, i am happy to provide a superbill for you to submit to your insurance for reimbursement, provided that you are able to give me a valid prescription from your doctor or prescribing entity PRIOR TO your appointment. please send all prescriptions and referrals to kyliecriswell.lmt@gmail.com.

  →Clients intending to bill insurance for their session MUST HAVE a prescription or referral PRIOR TO the scheduled appointment. Please send all prescriptions or referrals to kyliecriswell.lmt@gmail.com PRIOR TO your appointment time to avoid treatment delays or loss of hands-on time.

  →Copays for insurance depend on your particular insurance plan. I would encourage you to call your provider for coverage information in order to avoid any unwanted surprises!

clients intending to obtain a superbill are not eligible for sliding-scale rates at this time. Rates for superbills are as follows:

• 60m session — $120

• 90m session — $165
